​The sign-up form for Christmas gift box for children 12 and under is available to download and print.  If you live in the Luther area, volunteer at the Luther CSC or attend church with us and you have children, you are eligible to sign up. We are trying to get as many as possible signed up before 12 November. Gift pick-up will be on 18 or 19 December. Location and time to be determined.

You can print this form and bring the completed form to the Luther CSC, or you can obtain a form from the Luther CSC or the Luther Church of Christ.
